As a patient at Orange Regional Medical Center, a case manager will be assigned to assist you and your family by providing planning, coordination, and information to enhance the delivery of the healthcare services you require and to provide appropriate discharge planning. From the day of your admission, to the day you are discharged from the hospital, the Case Manager will work closely with your physician, the nursing staff, and other healthcare team members to determine your discharge needs.

The case manager can help you in a variety of ways:

Post hospital planning - by working with your doctor and other health providers to successfully prepare for your discharge and any necessary after-care. This could include home-care assistance, public health services or placement in a short-term (rehabilitation) or long-term care (nursing home) facility.

Problem solving - by helping patients and their loved ones deal with the impact of illness on their daily lives. This department provides information concerning community healthcare services and facilities supportive and protective services.
